Complete the following form/request to have your vendors ASUVPN Vendor account/certificate renewed. This form is only to be completed when your vendor requests their ASUVPN Vendor certificate/account to be renewed because it is set to expire or has expired.

If you wish to request a new ASUVPN Vendor account, please contact your Appalachian State University liaison/representative and have them request an account for your company. No new accounts will be created via this form, only renewed/reactivated.

*All ASUVPN Vendor accounts are subject to final approval by Network Infrastructure and Control Systems (NICS) and the department of Information Technology Servers (ITS).
